腎細(xì)胞癌（renal cell carcinoma， RCC）是腎癌中最常見(jiàn)的亞型，估計(jì)有40萬(wàn)例腎癌發(fā)生[1]。總體而言，男性患腎癌的終生風(fēng)險(xiǎn)約為1/46（2.02%），女性患腎癌的風(fēng)險(xiǎn)約為1/80（1.03%）[2]。近30%的患者為新發(fā)轉(zhuǎn)移性疾病，近30%的初始局部疾病患者在手術(shù)切除原發(fā)性轉(zhuǎn)移性疾病后最終會(huì)發(fā)生轉(zhuǎn)移腫瘤[2]。外核苷酸焦磷酸酶/磷酸二酯酶3（ENPP3）是腎細(xì)胞癌（RCC）中過(guò)度表達(dá)最多的表面蛋白之一，表達(dá)量高達(dá)94%[3]，是其成為抗體藥物的潛在靶點(diǎn)目標(biāo)。

外核苷酸焦磷酸酶/磷酸二酯酶3（ENPP3）是一種II型跨膜蛋白，其胞外結(jié)構(gòu)域包括一個(gè)核酸酶樣結(jié)構(gòu)域、一個(gè)催化結(jié)構(gòu)域和一個(gè)生長(zhǎng)調(diào)節(jié)素b樣結(jié)構(gòu)域。其主要功能是水解細(xì)胞外焦磷酸鹽或磷酸二酯鍵以及參與糖基轉(zhuǎn)移酶活性的細(xì)胞內(nèi)核苷酸[4]。ENPP3在腎細(xì)胞癌（RCC）領(lǐng)域也有較多的研究，有望成為潛在治療靶標(biāo)，在嗜堿性粒細(xì)胞和肥大細(xì)胞中的過(guò)敏反應(yīng)中有顯著作用，并作為潛在的過(guò)敏原敏感性的生物標(biāo)記物。

臨床進(jìn)展
Xencor的XmAb-819（ENPP3×CD3）是一款雙特異性抗體（TCE），可激活T細(xì)胞精準(zhǔn)清除ENPP3陽(yáng)性腫瘤細(xì)胞。根據(jù)2024年Xencor的官方信息，在晚期腎細(xì)胞癌（RCC）I期臨床中，已出現(xiàn)RECIST反應(yīng)等初步療效，部分患者治療持續(xù)超一年，安全性良好。
2023年12月14日，強(qiáng)生JNJ-87890387（ENPP3雙抗）已在多國(guó)進(jìn)入I期試驗(yàn)，用于實(shí)體瘤治療。
ENPP3正成為ADC新靶點(diǎn)布局熱點(diǎn)。國(guó)內(nèi)方面，先聲藥業(yè)SIM 0680已于2024年8月進(jìn)入臨床前研究階段。
